well how are you going to add it if the motor is down??

Motor has to be up to get to the fill!! There fore you fill when up ;)
 
Hmmm, maybe I've had too much of the kool-aid ;D but on my 88 Merc, the fill is through the port ? side engine bracket. You can get to the fill no matter what position the engine is in.

The engine has to be up on my 93 and 95 MERC's I have. My old 81 doesn't matter though, the pump is mounted inside the boat ;D.
 
I would think you would fill with the motor down.  The fluid in the reservoir would be at its maximum level.  If you fill with the drive up, it will overflow the reservoir as you lower the motor.  I don't know for sure, I've never had to top off. My pump is inside of the boat.
 
On the OMC the motor is up, and the fill hole is down a couple of inches from the top leaving you some room for the over flow when down
 
All power trim system's I've ever seen were designed to be checked with the engine/outdrive fully trimmed up. The fill plug is normally right at the pump's pickup level so that when trimmed up your almost out of fluid to help keep you from blowing your ram out of it's cylinder(no pum intended ;)). This also helps to ensure that the trim and tilt system stays under pressure to minimize the chance of water intrusion into the system. If there's pressure inside that means fluid can leak out, but water can't come in.
 
From my boat repair friend Jim:

fluid should always be checked with the motor in the full up position,
mostly because its easier and safer, as its a closed system, theres just as much oil on one side of the piston as there is on the other,and the level really dosent change,
besides on a johnson, you cant get to the screw to check it with the motor down,lol some yamahas and mercs you can though,
